How to register my baby at AZV?
As a parent you go to our website here at ‘Registration’ and choose to fill in the registration form. Answer all questions on the digital form completely, add a scan of your valid ID (valid passport or identification card or driver’s license) and a picture of your baby. We will process your registration and you will receive an email including instructions from us.
How do I change my family physician?
You can change of family physician twice in a year in the month of March and September. The procedure is as follows:
- Contact or go personally to the doctor of your choice;
- Ask them if they can take you as a patient;
- If the doctor agrees, you fill in the form and sign it at the office of your (new) doctor;
- The doctor also signs and stamps the form;
- The assistant sends the form to AZV thru [email protected] ;
- Or you can send it to us at [email protected] ;
- AZV processes the petition and you can see the change of doctor in your smartphone in the AZV app (in case you don’t have the AZV app, we invite you to download this from Google Play or Apple App Store).
How can I change pharmacy?
A change of pharmacy is only accepted when the request is made in the months of April and October. All other requests (made in other months) must be motivated and we will evaluate this. A request for pharmacy change can be send directly thru the AZV app (thru ‘Menu’ → choose ‘Your Service providers’ → choose ‘The Service provider’s of…’ → choose in that ‘Menu’ → ‘Change of Service Provider’ → Choose there the kind of Service provider you want to change and send the request).
Who is my Family Doctor?
Whenever you have the AZV app, you can go to the Menu → choose Service Providers of ….’xxx’ (choose the name of the Family member you want to see and the name of his or her doctor will appear).
Where can I find contact information of my family doctor?
You can find his name, telephone number and sometimes his emailadress in the AZV app under the Menu → Service Providers. On this website you can find a list of Health Service Providers, choose for the listo f Family physicians/Family doctors. Another option is by asking at your doctor’s office if they have a website or are on social media for you to find information.
Does AZV cover eye exams and or glasses?
AZV covers eye examinations that are prescribed by your Family doctor or specialist and which takes place in the Eye Center of ImSan or at the office of doctor Burton Jacobs. AZV also covers eye exams performed by a optometrist thru a referral letter from your Family doctor. Glasses are covered when there is a dioptre of 10>.
Does AZV covers pshycology?
No. And this is not because AZV doesn’t believe in pshycology sessions. This is due to financial factors. In the past, when preparing the Law AZV choices had to be made upon the AZV package. It was at that time that the decision was made to exclude pshycology.
What is covered by AZV?
- Care from a Family doctor.
- Care from a specialist.
- Medicines (according to the Positive List).
- Hosital care.
- Medical devices.
- Medical care abroad (with a approval from AZV).
- Midwife care.
- Physiotherapy.
- Dental care (very limited coverage).
What isn’t covered by AZV?
- OTC (Products or medicines ‘Over The Counter’).
- Dietary supplements or similar.
- A general preventive checkup.
- A driver’s licence exam.
- A company exam related to a job application.
- A medical exam related to a request for a DIMAS permit.
- Organ transplant (except for cornea, bone marrow and kidney)
- Treatments or operations in experimental phase.
- DNA testing related to paternity.
- Death certificate.
I have lost my AZV card. How much does it cost to obtain a new card?
It is free. We don’t use plastic cards anymore. Now you can download the AZV app and register for your digi-ZorgPas (the digital AZV card in your smartphone).
What do I have to do when the AZV app tells me my ID number is invalid?

I have forgotten my pincode. What do I have to do to log into the AZV app?
After 5 attempts, the system will automatically reestablish itself by allowing you to reset with a new pincode. In case you don’t succeed, we recommend you to contact us by sending an email at

Can I still use my AZV card (ZorgPas)?
Yes. Now you still can. But take notice that whenever you are not insured with AZV, you can’t use the card. For example, the moment you deregistrate from Census and go abroad to live. It may be possible that you still have the AZV card, but when returning to Aruba you won’t be able to use this because the service health provider will be checking into the AZV webportal to verify whether your are insured or not. Everytime you leave Aruba and then return, you will have to register again.
How can I add my children to my AZV app?
Parents can add their minor children thru Menu → Service Health Providers → Add New Account, here you fill in the information of your child (AZV number, family doctor and the last two digits of the identification number of your child). The parents that are registered with us as ‘verified user’ can add their child thru a QR code, the moment they request a QR code at our email

I have submitted all my documents at DIMAS but still the AZV app indicates ‘invalid’. What do I have to do now?
You can download the AZV app and log in. For the prolongation (renovation) of your AZV insurance you will have to sent us a request -including a scan of a valid ID and a recent picture- at our email
[email protected] Once we have processed your AZV policy, you will see this in the AZV app. And by having the notifications button in the AZV app on, you will receive a notificacion or an email.
I have a new mobile phone and would like to have the AZV app back.
Through your old mobile phone you can register again on the new one. If you know your AZV number you can register yourself on the new mobile phone, or you can also request us for a QR code (by sending an email requesting a QR code at

Why do I have to lock my mobile phone?
Because of privacy of your medical personal data.
