Change your family physician
If you want to change your family physician, please note that not all physicians have space for new patients. You can check this list to see which practices are open for new patients; these are indicated in green. You can visit the care provider and make the change there. Your new doctor will inform AZV of the change, and we will update your policy information. That change will be visible in the AZV app.
